Review the Medical Questionnaire
Before confirming your diving course, we ask every student to review the official PADI Medical Questionnaire. This allows us to confirm you’re eligible to participate before you pay your deposit or begin your PADI eLearning.
There’s no need to complete it now—simply review it to make sure you’re eligible to dive.
Before Booking:
1 · Review the questionnaire
Read the official PADI Medical Questionnaire carefully before booking.
2 · If medical clearance is required
If you answer “Yes” to any question requiring medical approval, download the questionnaire below and have the Physician’s Evaluation completed before your course.
3 · Book your course
Once we know you’re eligible to dive, we’ll confirm your reservation and send your PADI eLearning and online registration.
4 · Complete your online forms
After booking, you’ll complete the official online PADI medical declaration and student registration before arrival.
Most students are immediately eligible to dive. Only some medical conditions require approval from a physician.
Don’t worry: answering “Yes” to one or more questions doesn’t automatically mean you can’t dive. In many cases, it simply means you’ll need approval from a physician.
